Post-acute care (PAC) plays a critical role in patient recovery and chronic condition management, evolving with trends like value-based, technological advancements, and workforce challenges. Payment model shifts, including accountable organizations, are transforming PAC by promoting efficiency and coordination among providers. The emphasis on value has led to more patients receiving home health instead of skilled nursing facilities, supported by telehealth and remote monitoring. Patient-centered is gaining traction, improving satisfaction and outcomes by involving patients in decision-making. Technology, such as EHRs and AI, enhances care coordination and decision-making, while workforce shortages in skilled professionals push organizations to invest in training and retention. Regulations ensure transparency in quality metrics and patient outcomes, and social determinants of health (SDOH) are increasingly incorporated into plans, improving patient.
