The analgesic effect of buprenorphine is attributed to partial agonist activity at -opioid receptors, which means that when the molecule binds to a receptor, it is only partially activated, as opposed to a full agonist like morphine. Because buprenorphine has such a high affinity for the receptor, opioid receptor antagonists (such as naloxone) can only partially reverse its effects. Buprenorphine is delivered via transdermal patches in doses of 35, 52.5, and 70 mcg/hour for a total of 96 hours. Because of the high first-pass metabolism, it is not given orally.
